Largest-Yet Mesothelioma Study Shows Survival Benefit with New Drug

Researchers with the largest phase III trial to date for mesothelioma, an aggressive cancer affecting the lining of the lung, reported results showing that patients on a new chemotherapy drug regimen live longer and have less pain than those on an older drug. The findings were announced at the annual meeting of the American Society of Clinical Oncology meeting in Orlando, Fla., on May 20, 2002. (NOTE: The final data were subsequently published in the July 15, 2003, issue of the Journal of Clinical Oncology; see the journal abstract).

Pemetrexed (brand name Alimta™) is a novel antifolate, a class of drugs that targets the folic acid metabolic pathway, which effects availability of certain B complex vitamins. The results of the trial show that tumors shrank in 41 percent of patients on pemetrexed in combination with a more commonly used chemotherapy agent called cisplatin. Only 17 percent of patients receiving cisplatin alone experienced tumor shrinkage. Additionally, those on the pemetrexed combination lived nearly three months longer than those on cisplatin alone.

According to lead author, Nicholas J. Vogelzang, M.D., University of Chicago Cancer Research Center, "This is the largest clinical trial ever conducted in this disease and the 25 to 30 percent improvement in survival for patients on the combination therapy is the first time anyone has documented a significant improvement in patients treated for mesothelioma."

Malignant pleural mesothelioma is associated with a history of asbestos exposure in about 70 to 80 percent of all cases and there is no approved or very effective chemotherapy for the disease. Researchers hypothesized that pemetrexed might prove effective in treating this disease because it targets key enzymes (molecules that speed up chemical reactions in the body) thought to play a role in allowing the rapid growth of this tumor.

Early phase I trial results in 11 patients tested with pemetrexed and cisplatin were promising and a definitive randomized phase III trial was developed. Since there are no established therapies for this condition, a standard chemotherapy agent called cisplatin that has shown efficacy in treating other diseases, was used as the control group. The phase III study initially planned to enroll 456 patients from April 1999 to March 2001. However, after enrolling 150 patients, a high rate of severe toxicity and death was associated with the pemetrexed and cisplatin arm of the trial. Elevated levels of homocysteine, a chemical byproduct that results when proteins are broken down in the blood, were found, which provided a basis for redesign of the trial to reduce the dangerous drug side effects.

Two hundred and eighty patients were enrolled to the revised protocol. Using a strategy to reduce drug side effects that has been successful in the past, this new protocol added folic acid to the regimen because pemetrexed as an antifolate agent reduces levels of this important vitamin. Folic acid was given prior to and during the trial, and vitamin B12 was given only during the trial. Both vitamins should boost folic acid levels, reduce homocysteine formation, and hence reduce toxicity to pemetrexed. "We now have a significantly less toxic regimen than the one we started with," said Vogelzang.

Because of the presumed importance of the vitamins to the study, the researchers examined not only the combination therapy versus the single drug therapy, but also looked at the results of patients on the vitamin supplements versus those early enrollees who had not initially received vitamins.

Standard treatment for malignant mesothelioma has been surgery. Surgical treatment rarely results in cure and long-term survival is unusual. Use of radiation therapy and/or chemotherapy following surgery has not improved survival for patients but radiation treatments may alleviate some pain associated with the disease

Watchuk found Moore would get his vulnerable, young workers to remove asbestos from jobs he had successfully bid on "without providing protective equipment to his employees."

Asbestos was a popular building material for its fire-retardant properties up to the 1970s. That changed when it was determined asbestos is a slow killer, claiming its victims 10 to 20 years after they were exposed to the substance.

It is the biggest cause of workrelated deaths in B.C. -53 of the 121 death claims in B.C. in 2009 were asbestos-related.

Although Watchuk found Moore "poses a significant public safety concern" and "targeted ... vulnerable workers," she dismissed the application to have him jailed.


Some of Moore's workers were as young as 14, the judge heard.

Moore had told his employees to run if any WorkSafe B.C. personnel appeared on site, one witness testified.

With no assets -not even a driver's licence -Moore, who has used a number of aliases, has escaped punishment for his misdeeds. He did not show up at the two-day hearing before Justice Watchuk.

Al Johnson of WorkSafe B.C. said people offering to do asbestos inspections and removal aren't required to be licensed. It's buyer beware, he said, noting homeowners should check references.

"Asbestos materials in place, for the most part, aren't dangerous. It's when they are disturbed and the fibres get into the air [where they can be inhaled]. "You can't see them, you can't smell them, you can't taste them," he said. "The fibres go deep into the lungs."

Despite its established threat to human health, asbestos was once extensively used in construction and manufacturing projects. Although asbestos use is now firmly regulated in the United States, construction projects are often conducted at sites where the mineral naturally occurs or at buildings where asbestos products were used. Sampling tests are commonly used to estimate the amount of asbestos disturbed by the construction as well as the health risk posed to workers.

Samples taken from asbestos worksites are most commonly used to assess the risk for construction workers, individuals who pass through the worksites during construction, workers employed at the finished worksite and residents who live near the area. Calculations project the risk of developing an asbestos-related illness such as malignant mesothelioma, as well as absolute risk, which is the chance of death from a complication of cumulative asbestos exposure.

Risk assessments for asbestos-related diseases are based on the level and extent of a person’s asbestos exposure. Extent includes the number of hours per day, days per year and total years that were spent in an asbestos-contaminated worksite. While extent varies from person to person, the precise level of exposure at a given worksite is determined by analyzing an air or soil sample.

The soil collection method currently used is the Berman and Crump approach. During this process, a soil sample is suspended in a machine called an elutriator. The modified elutriator method, adopted in 2000, uses the number of overall asbestos structures in the soil to also anticipate the quantity of airborne asbestos.

In September 2008, a new air sampling approach was introduced to more closely reflect the conditions created by the actual construction process. This approach analyzes breathing zone samples taken after soil is mechanically disturbed. Known as activity-based sampling, this method more adequately reflects the risk of asbestos inhalation during common construction activities.

After sampling has been conducted, data quality must be considered and reports must be checked for completion. This process ensures the submission of an accurate report for publication.

Patients with non-epithelial mesothelioma and lymph node involvement are poor candidates for the extensive surgical procedure known as extrapleural pneumonectomy (EPP), according to a new study.

That’s the finding of a new article published in the Annals of Surgical Oncology. Researchers from the Baird Institute for Applied Heart and Lung Surgical Research in Sydney, Australia analyzed the current literature on EPP for malignant pleural mesothelioma. They examined the patient selection process and overall survival of surgical patients in major referral centers where EPP is performed.

The most serious of the asbestos cancers, mesothelioma tends to spread quickly and is notoriously resistant to standard treatments. Clinical trials across the globe have confirmed that the best mesothelioma outcomes are seen in patients who undergo EPP, followed by adjuvant therapies such as chemotherapy and/or radiotherapy.

In the new review article, seventeen studies from 13 institutions were found to offer the most complete and up-to-date information for analysis. The authors report that “a number of quantitative, clinical and treatment-related factors were identified to have significant impact on overall survival.”

While the researchers found marked differences in the way different institutions select mesothelioma patients for surgery, they found that, across the board, patients who had sarcomatoid or biphasic mesothelioma did not fair as well after surgery as those who had the epithelioid type. Likewise, mesothelioma patients whose cancer had spread to their lymph nodes also did not do as well after surgery as patients whose tumors were confined to the mesothelium. In their conclusion, the researchers suggest that these patients be excluded from consideration for EPP.

EPP, which involves removing not only the cancerous tissue but, often, a lung or portion of a lung, carries serious risk of complication or even death. Careful selection of mesothelioma surgical candidates, say the study’s authors, not only improves outcomes for the right patients, but protects those who are not good surgical candidates from undergoing a risky operation for limited benefit.

Researchers at the University of New York Langon NYU Medical Center and Cancer Institute in New York has resulted in trials at the National Cancer Institute Early Detection Research Network Biomarker Discovery Lab with what they now call the biomarker most accurate in detecting proteins secreted by tumors asbestos cancer. Scientists have found that the proteomic analysis detected 15 of 19 cases of malignant pleural mesothelioma who were in phase 1 or phase 2. Mesothelioma is usually very difficult to detect and is usually diagnosed only when it becomes symptomatic around the scene three or four, which makes it difficult to treat and almost impossible to beat.

"Only patients who seem to benefit from treatment of mesothelioma are those who are in Stage 1 and this is only 10-15 percent of patients," said the head of the Division of Thoracic Surgery and Thoracic Oncology at the Langone NYU Medical Center, Harvey I. Pass, MD, who believes that a diagnostic tool could be a turning circuit breaker.

Approximately 70 students have been exposed to asbestos in a school in West Berkshire, England, after the explosion damaged a ceiling a science classroom. Pleural mesothelioma

Radiation therapy do not cause any harm to the healthy cells. Chemotherapy is one of the common treatments for pleural mesothelioma. In this treatment high power drugs are given to the patients either by injecting it or in the forms of pills. These drugs may cause some side effects such as vomiting, hair loss, nausea, weight loss and fatigue. The disadvantage in this treatment is that the drugs administered have to make their way to the cancerous cells, unlike radiation therapy where the radiation is directly targeted to the affected area. This may cause harm to the healthy cells as well.

Palliative therapies involve extracting extra fluid by needle and suction from the affected areas. In order to keep fluid accumulation under control, drugs are used. Researches are being performed to find new ways to treat this deadly cancer and so you should always keep yourself updated with these treatments and researches.

Palliative care is a contemporary and advanced method of treating and helping the patients diagnosed with fatal diseases. This treatment endeavor to ease the psychological problems related with the illnesses.

Mesothelioma

Mesothelioma is a slow growing disease and cannot be diagnosed in the early stages. The symptoms are also very common which are to be mistaken with some other disease. The common palliative procedures for the treatment of mesothelioma are Chest Tube Drainage and Pleurodesis, Pleuroperitoneal Shunt, Alcohol injection, Palliative radiation therapy and Pain medications. In Chest Tube Drainage and Pleurodesis the fluid built-up due to pleural effusion, one of the symptoms of mesothelioma, is drained out of the chest cavity with the help of chest tube. The pleural space is closed by use of talc slurry or other agent which yields an adhesion.

Pleuroperitoneal Shunt plays a partial role in palliation. It involves placement of a catheter run under the skin from the pleural to the peritoneal cavity. Alcohol injection eases the pain caused during the surgery. This inactivates the nerves that pass the message of pain from the affected area to the brain. Pain medications include strong pain-killing drugs which reduce pain. This also includes narcotic drugs, which are non addictive.

Palliative radiation therapy reduces the pain using radiation therapy as it kills the cancer cells. In this therapy, patients are given larger doses of radiation over a period of several weeks. Many patients have experienced good results with this treatment. These are some of the palliative treatments to be considered during the treatment procedure of mesothelioma.
